The following link is CORI Form, to be completed for any Coach or assistant coach within the MMC Youth Football and Cheer Program.
CORI Forms will be completed and emailed to mmcfootball@verizon.net, with Subject Line of CORI, followed by name and team you are coaching. All CORI's must be submitted prior to any participation as a volunteer coach or assistant coach.
All information MUST be accurate and complete.
